Your admission to MHPH will be arranged by your treating Doctor. Once it is confirmed that you need to be admitted to hospital, your Doctor will provide you with admission paperwork to be presented to the Hospital.
- For assistance with your admission paperwork, please contact the MHPH Reception team on 0350222611 Monday to Friday between 6.30am and 8pm or 8.30 am – 5.00 pm on Saturday and Sunday.
- Any queries of a medical nature should be directed to your treating Doctor.

What do I need to do before admission?
Please ensure that your Pre-admission Planning form is completed and returned to the hospital as soon as possible. To allow sufficient time for all paperwork to be processed by our Hospital staff, all paperwork should submitted at least 2-weeks prior to your admission date.
Where do I report for my admission?
Please present to the Main Reception area of the hospital.You will either be directed to the Day Procedure Unit or admitted through to the Banksia Inpatient Ward.
